All 3-sample types are required. Submit pink and lavender top unspun. Blood Bank (pink and lavender tubes) samples must be labeled with the date collected.
Tests included are: Blood bank Prenatal Study, Hepatitis B Surface Antigen, Prenatal, Syphilis Serology, Rubella IgG Antibody, and Hemagram with Differential (Includes: WBC, RBC, HGB, HCT, indices, PLT), AST, ALT, BUN, Creatinine, Uric Acid ,  and  Varicella IgG Antibody .

When ordering a custom panel, Medicare requires the clinician confirm each test in the panel is medically necessary for the patient’s treatment. By regulation, annually we will send a physician acknowledgement letter to ordering clinicians requesting a signature attesting to medical necessity review.
